Checklist: Required items for 2026/2027 registration at Lighthouse Preschool, including a photo, birth certificate, specific CFS forms, a non-refundable fee, session preferences (2-, 3-, or 4-day options), and payment choices for monthly or annual tuition.

Child Record: This form collects essential information about the child, including personal details, parent or guardian contacts, emergency notifications, physician information, program schedule, and additional notes on medical issues, restrictions, allergies, and care preferences to ensure proper childcare.

Consent: This consent form authorizes the daycare provider to handle emergency medical care, administer prescription or over-the-counter medications, allow specified individuals to pick up the child, permit participation in trips and excursions, and approve swimming activities, all signed by parents or guardians.

Physical: This certificate of child health examination documents the child's immunization history, alternative proofs of immunity, and requires verification by a healthcare provider to confirm the child's medical fitness for enrollment in licensed childcare facilities.
